By supporting the Myeloma Institute you can show others that you not only value us as an employer, but also as a leader in advancing myeloma healthcare to improve the lives of our patients worldwide.  Gifts of all sizes truly make a difference.

The easiest way to participate in the employee giving program is through payroll deduction.  The online form ensures that the information you submit will be processed safely.  You will receive an email confirmation after setting up a payroll deduction gift.  Deductions begin within 30 days after the form is processed.  You can change your payroll deduction at any time.

If you wish to download and complete a printed payroll deduction form  please give the form to Jennifer Gurley, Senior Director of Development.

If you would like to make a one-time gift by check, complete the standard gift form and give to Jennifer Gurley.  All checks should be made payable to the U of A Foundation, Inc.  To make a one-time gift by credit card, please complete the online gift form
